Medical providers terminated or excluded from Washington State's Medicaid program.

This dataset includes individuals or entities whose participation in the Medicaid program has been terminated for cause under the provision of WAC 182-502-0030 (HCA) or 388-113-0020; 388-113-0030; 388-71-0540 (DSHS) and Medicaid providers who have been excluded from participation (see 42 CFR 1001).
